Guidelines


[New application] for general food preparation facility

Please submit the following documents approximately 10 days before the scheduled completion of your facility.


☆Required documents

1 Business license application form (PDF:225KB Excel:43KB)
2 Current business license certificate (floor plan showing the structure and equipment of the facility should be attached)
3 Certificate verifying the food sanitation manager (food sanitation manager license book etc.)
4 Water quality examination results conducted within12 months prior to application (in cases using water other than tap water, private water supply system (専用水道), or private water supply facility(簡易専用水道))
5 Application fee (PDF:103KB)
(in cases of incorporation)
The authenticity of the corporate registry will be confirmed by the corporation number on the business license application form. Therefore, if you do not submit the corporate number on the business license application form, Certificate of Registered Matters of the corporation (登記事項証明書) should be attached.

[Renewal] for general food preparation facility

If you wish to continue operating after your business license expires, please apply for a renewal approximately one month prior to the expiration date.

When any change occurs in the filed information on the business license application form or in the operational facility, please notify such effects within 10 days of the change.

Enter the corporation number in the form to confirm the authenticity of the corporate registry.
If you do not submit the corporate number in the business license application form, the Certificate of Registered Matters of the corporation (登記事項証明書) should be attached.

Depending on the extent of the changes and the circumstances, a new business license application may be required. Please consult in advance.

In above cases, please notify any closures within 10 days from the change.

※In case of (2) and (3), you must apply for a new business license. However, concerning (3), cases due to transfer, inheritance, corporate merger, or corporate divestiture, succession of the business license may be permitted. Please consult in advance.

To initiate a succession of the business license on any of the following grounds and to continue operating the business, please submit the notification of the succession without delay. If a transfer is planned, please consult with us in advance.

3 Documented evidence of the relevant facts

  • Written consent of all heirs (if there are two or more heirs)

* When the business was succeeded through a transfer, an investigation will be conducted by a local health center about the situation of the business.

Food sanitation supervisor

Preparations

Business operators engaged in the manufacturing or processing of the following items are obligated to allocate a food sanitation supervisor. When a food sanitation supervisor has been allocated (or changed), please submit the notification within 15 days.


  • Whole milk powder (confined to products contained in cans with a capacity of not more than 1,400g)
  • Sweetened milk powder
  • Formulated milk powder
  • Processed meat products (ham, sausages, bacon, and similar products)
  • Fish ham
  • Fish sausages
  • Irradiated food
  • Edible fats & oils (confined to products manufactured through a process of decoloring or deodorizing)
  • Margarine
  • Shortening
  • Addictive (confined to products for which specifications have been established pursuant to the provisions of Article 11 Paragraph 1 of the Food Sanitation Act)

3 Document certifying the fulfillment of the items in Article 48, Paragraph 6 of the Food Sanitation Act.
